• Full name (as in passport)

• Photo/scan of the first page of the passport (clear image)
• Completed official insurance application form (see Step 2)
1. Get the official application form. Obtain the CAMT official insurance application form from the CAMT office/insurance coordinator or fill it online.
2. Fill the form. Complete the form legibly and accurately with the required information (name, student ID, major, passport number, contact info).
3. Prepare passport image. Take a clear photo or scan of the passport’s first page (full page, readable).
4. Attach supporting documents. Attach the passport first-page image and any other requested documents to the form.
5. Submit to the designated collector (If in person). Give the completed form and documents to the CAMT insurance coordinator / designated staff person (the person in charge who collects applications). Make sure you know the office location and office hours.
6. Coordinator uploads to insurer. The CAMT coordinator will submit/upload the student’s information and form into the insurance company’s system.
7. Company issues insurance card. After processing, the insurance company will create the student’s insurance record and produce an insurance card.
8. Distribution of cards. The CAMT coordinator will receive the cards from the insurer and distribute them to students. Confirm how and when distribution happens (picked up in office or handed out in class).
9. Keep copies. After you receive your physical insurance card, keep it safe and also keep a photo/scan copy for your records and for online claims.
10. Read policy basics. Review the card/policy for coverage limits, exclusions, and claim procedures so you know what is covered.
Notes / Tips:
• Use a clear, readable passport image (no glare, all corners visible).
• Ask the CAMT coordinator for an estimate of how long processing/distribution will take.
• Keep a record (email or receipt) that you submitted the application.
